Mammogram Screening at Health Fair

Call (847) 455-8616 to schedule a mammogram. 

Mammogram Requirements: 

Age Requirement: Must be at least 40 years old.

Previous Mammogram: Must be greater than 365 days from last mammogram.

Pregnancy & Breastfeeding: You cannot be pregnant or currently breastfeeding.

Current Breast Health: If you have no new problems or concerns with their breasts and have not had a cancer diagnosis within the past two years.

If you are experiencing any problems such as a lump, pain, or discharge, you will be unable to receive a mammogram in the unit.

Photo ID, Insurance, and established PCP information.

PCP information is required in order to send the results for review. This information is important should a patient require additional follow-up or intervention.
